Job Description

Woodward Resource Center (WRC) is seeking a Licensed Practical Nurse (LPN) to join our Team. This job is located in Woodward, Iowa. Woodward is 35 minutes away from Ames and 34 minutes to Des Moines.

Health care problems for individuals will be assessed and documented according to policy and procedures. This includes, but is not limited to completing Incident Reports, neurological checks, PNM nursing assessments, vital sign monitoring, and illness/professional contact related event logs. Event log entries will be documented using SOAP (Subjective Objective Assessment Plan) method of charting. The health care provider will be contacted per protocol or health care need.

Emergency Equipment will be checked monthly according to WRC policy and procedures. Will perform preventative health care practice which includes, but is not limited to: glucose checks, immunizations, routine scans and x-rays, and process routine lab orders.

When a health care need is identified, the LPN will work with the RN and the IDT to provide appropriate nursing care and supports. Will prepare for and participate in all annual ISP meetings, Monthly Integrated Reviews, and Quarterly Psychiatric Treatment Reviews for assigned units when the RN is absent. Will provide assessment, and monitoring of self-administration of medication and medication administration procedures. Will assist in completing training on all new and transfer employees on Health and Safety Support Plans. Will participate in peer review as assigned unless otherwise excused, by giving constructive feedback/ideas on areas of improvement.

The LPN will enter monthly weight data in the electronic charting system-IPR. Will ensure new monthly Medication Administration Records are complete and accurate. Work with the IDT on any problems identified on pharmacy round inspections. When problems are identified, will work jointly with the IDT to implement corrective action.

All nursing medications, treatments, and procedures will be performed according to policy and procedures and will be documented by the end of the shift. The LPN will be responsible for performing weekly checks on the MARs and Treatment records of their assignments and addressing any discrepancies according to policy and procedure.

All chemical and physical restraints will be monitored according to facility policy. All PRN medications will be co-signed when being reviewed by the nurse every month. All pharmacy orders will be noted accurately according to WRC policy and procedure. All health care revisions including medication changes will be reported and documented via the interdisciplinary team communication system. Required DAR information will be reviewed daily, and appropriate action taken for identified triggers. CMA medication observations will be completed as assigned. Med carts and storage areas will be inspected monthly, cleaned if needed, and expired drugs removed.

Will conduct all interactions in a professional and courteous manner. Attend and participate in mini staffings or mini reviews, department meetings, and weekly team communication meetings unless otherwise excused. Complete information for all off campus facility transfers and Home Visits according to WRC policy and procedures. Prioritize daily work assignment according to severity of conditions.

Safety and health hazards shall be identified with appropriate follow up documentation, training and communication according to WRC policy and procedure. Will participate fully in all code blue drills. Will complete Health Care Provider CPR every 2 years. Will use universal precautions, procedures, and equipment 100% of the time for all anticipated contacts with blood and body fluids. Will follow reporting procedure 100% of the time for all actual exposures to blood and body fluids. Complete duties according to instructions given at the time of the assignment.

Licensure as a Licensed Practical Nurse by the Iowa Board of Nursing or licensure as a Licensed Practical Nurse in accordance with the Nurse Licensure Compact Administrator's Agreement of the National Council of State Boards of Nursing.
