We have amultiple full-timepositions available for aLicensed Practical Nurse (LPN) or Registered Nurse (RN)at ourFamily Practice ClinicinWest Des Moines, Iowa. UCS Healthcare is a CARF-accredited, non-profit health care organization based in West Des Moines, Iowa with additional offices in Knoxville and Ankeny. UCS also operates 11 medication units across the state offering medication assisted treatment (MAT) for opiate/alcohol addiction.We offer a competitive hourly wage and benefits package.

Job Summary:To provide comprehensive nursing/medical care to clinic patients by cooperating with other medical professionals in a team environment while maintaining professional nursing/medical standards.This position will primarily assist with our family practice care, but responsibilities or coverage can include any of our practice areas.
As part of UCS Healthcare, team members will follow all laws and regulations, including those at the Federal, State, and Local level. They will also adhere to all UCS policies and procedures, including patient confidentiality, HIPAA/ 42CFR part 2, and all CARF requirements. All team members will operate in alignment with UCS Healthcare\'s core values, work effectively in a team environment, provide or support patient-centered services, and strive to assist UCS Healthcare in delivering the highest level of care.
Essential Functions:Includes but are not limited to:
Communication with stakeholders involved in patient care. (40%)

- Triage phone calls appropriately.
- Appropriately schedule patients within physicians requested parameters.
- Accurately calls in/e-scribes medications in a timely manner.
- Demonstrates competence in specified computer applications.
- Meets consistently the clinic work schedule as designated.
- Demonstrates consistently a team approach to care.
- Demonstrates effective communication strategies.
- Adheres to all policies and procedures.

Documentation (30%)
- Maintains the standards of nursing care and implements the standards, policies, and procedures of UCS Healthcare and the Code of Iowa.
- Performs screening, assessment, and data collection in an ongoing and systematic manner.
- Implements care in a knowledgeable, skillful, consistent, and continuous manner by utilization of pre-determined standards and evidenced-based practices that define the structure, process and desired patient outcomes.
- Competes Prior Authorizations and referrals in a timely manner.
- Performs efficiently in emergency situations following established protocols.
- Implements educational materi ls and evaluates their effectiveness.
- Participates in orientation of new personnel.
- Participates in development and attainment of UCS Healthcare goals.
- Participates in agency meetings and conferences for the betterment of patient care.
- Communicates effectively, practices effective problem identification and resolution skills and uses lines of authority appropriately.
- Document program specific data collection (Methasoft)
- Document program specific data into CareLogic
- Implement and maintain Opioid Treatment Standards within CARF and state. Federal and agency rules and regulations.
Patient Care (30%)
- Performs venipuncture while maintaining safety standards.
- Promotes nursing practices and patient care that is effective in terms of quality and cost, and results in patient satisfaction.
- Rooms patients and accurately assesses vital signs (blood pressure, respiratory rate, pulse and temperature) and reason for visit.
- Maintains a safe and secure environment for patients and staff.
- Administers medications and evaluates their effectiveness as well as any adverse reactions.
- Participates in orientation of new personnel.
- Scanning and uploading documents
- Inform nursing staff of patient needs.
- Coordinate the functions to maintain and distribute accurate medical records.
- Plants and reads TB tests.
Minimum Required Knowledge, Education, Experience, Skills, and Abilities:
- Unrestricted license in the state of Iowa as an LPN or RN. CPR certificate.Venipuncture skills required.
Preferred Knowledge, Education, Experience, Skills, and Abilities:
- Previous medical office experience.
Physical requirements:
- A valid driver\'s license is required (an Iowa driver\'s license is required within 30 days of hire) and liability insurance or an equivalent means of personal transportation and coverage as travel within the state of Iowa may be necessary.
- Communicating with others to exchange information.
Work Schedule:Full-time, Monday - Friday 8:00 AM - 5:00 PM
