Description

SUMMARY

Provides pharmaceutical care services to WCHC patients requiring specialized clinical pharmacy knowledge and practice expertise.

DUTIES/RESPONSIBILITIES (include but are not limited to the following)

  • Provide pharmaceutical services to the Inpatient department (22 bed) and to the Out Patient Infusion Center.

  • Evaluation of medication therapy, clinical assessment of patients, medication reconciliation, medication management, pharmacokinetic consults, resolution of emergent lab values, management of adverse drug reactions, therapeutic drug monitoring, and discharge planning.

  • Dispense and compound medications, including parenteral , and/or supervises this process.

  • Provide accurate, efficient and appropriate pharmaceutical distribution services to patients by properly interpreting providers drug orders and dispensing the appropriate medications.

  • Communicate with after hours pharmacy service staff to ensure transition of care.

Qualifications

EDUCATION AND/OR EXPERIENCE

  • Pharm. D. or B.S. from an ACPE-accredited College of Pharmacy, is required.

  • An ASHP-accredited residency in clinical/hospital pharmacy, or equivalent experience, is highly desired.

  • Current licensure as a pharmacist, by the Iowa State Board of Pharmacy, is required.
