Job Description

We are seeking a dedicated and knowledgeable Registered Dietitian to provide comprehensive nutrition education and counseling to patients with kidney disease. The ideal candidate will collaborate with an interdisciplinary team to develop individualized care plans, assess nutritional status, and ensure effective management of diet and fluid intake.

Responsibilities

• Educate patients on diet, kidney disease, and the impact on their health.

• Provide detailed education on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters.

• Complete nutritional assessments and participate in interdisciplinary comprehensive assessments.

• Work with the interdisciplinary team to develop individualized care plans addressing patients' needs.

• Calculate diet prescriptions according to standard practice.

• Document notes on chronic dialysis patients following guidelines.

• Maintain complete and accurate medical records as per policies and regulations.

• Provide nutrition assessments and outcome-oriented counseling to help patients achieve effective nutritional status.

• Identify malnourished patients and collaborate on recommendations for their care.

• Participate in Urea Kinetic Modeling recommendations and collaborate on management for mineral metabolism, renal bone disease, anemia, and other related conditions.

• Report on nutrition QAI results and participate in the QAI program.

• Coordinate patient education with case managers and refer candidates for nutritional supplements.

• Review and communicate nutrition lab results and plans with staff.

• Maintain dietetic registration and continuing education as required by relevant associations and regulations.

Pay and Benefits

The pay range for this position is $28.00 - $32.00/hr.
