** This position is eligible for a $5,000 sign-on bonus.

Under limited supervision, and as an integral member of the pharmacy team (under the supervision of a pharmacist), is responsible for working with newly admitted patients to obtain and document an accurate medication list to help avoid medication duplications, omissions and errors. The information obtained will be entered into a computer system.

What you will do:

  • Gathers information about the patient’s medications directly from the patient or other source in a professional manner.

  • Collects and compiles a complete list of patient’s home medications including over the counter medications and herbal supplements. List includes drug name, strength, frequency, compliance and last dose taken. This may require collaboration with alternative information sources such as family, physician office, nursing home or community pharmacy.

  • Interviews to obtain any known drug allergy and reaction.

  • Contacts pharmacies, physician offices, etc, to obtain the most accurate medication list possible.

  • Communicates effectively using advanced interviewing skills to conduct the medication history interview with patients in a professional manner.

  • Inputs all medication history and allergy information into the electronic medical record in an accurate and timely manner.

  • Demonstrates proficiency in computer skills for area of responsibility.

  • Problem solves and applies critical thinking skills in obtaining admission medication lists for a wide variety of patient populations.

  • Reports suspected medication errors or adverse drug reactions to the supervising pharmacist.

  • Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control Policies and Procedures.

  • Maintains job-related skills and educational development and assists in the education of other team members.

  • Participates in performance improvement activities.

  • Knows, understands, incorporates, and demonstrates the MercyOne and Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.

What we are looking for:

  • Must a high school diploma or equivalent.

  • Current registration as a pharmacy technician in the state of Iowa or eligible. This must be completed within 30 days of employment.

  • Pharmacy Technician Certification preferred upon hire but required within one year of hire into this position.

  • Previous experience as pharmacy technician (2 or more years community pharmacy experience preferred), with knowledge of medications including brand/generic antes, available dosages, frequencies, and indications.

  • Excellent written and verbal communication skills.

  • Requires close attention to detail, ability to work with others, and problem-solving skills.
