Description:Under direct supervision, performs computerized patient scheduling, medical record filing and management, medical record chart prep, data entry, answers telephone calls from patients. Completes the patient office visit, which includes registering patients, receiving payment on patient accounts, and scheduling follow up appointments. What you will do:

  • Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards.

  • Files and retrieves patient medical records. Maintains adequate filing and retrieval systems to assure prompt availability of records.

  • Files all reports received into the appropriate patient chart.

  • Maintains the medical record system for accuracy and timeliness of service.

  • Maintains the patient file by ensuring transcription and/or notation is made for each office visit, hospital visit, outreach visit and etc.

  • Assists with copying of patient charts for insurance companies and for patients at their request for transfer of records following established policies and procedures.

  • Checks in patients following Standard Work Instructions (SWI).

  • Updates and verifies patient demographics, making appropriate changes as needed following SWI.

  • Meets all information management, HIPAA, HITECH, confidentiality and patient rights standards.

  • Enters the room with the physician, identifying the patient with two patient identifiers.

  • Assists the physician during the appointment by documenting only information cited by the physician, refraining from obtaining any information from the patient directly without the direct supervision of the physician.

  • Records details of chief complaint, history of present illness, review of systems, past medical history, social history, family medical history, disease risk factors, medications and allergies.

  • Records details of physical exam, including pertinent positives and negatives, abnormalities and patient general condition.

  • Submits orders as desired by medical provider and check for results of those orders.

  • Remains in compliance with the Record of Care and Provision of Care Standards Provides statement:

  • Identifying the scriber of the service.

  • Statement attesting that the notes are written/recorded contemporaneously in the presence of the physician performing the service.

  • Physician name.

  • Signature (including name and title), dating and timing of all entries into the medical record (Example: Scribed for Dr. X in their presence by Susie Scriber, MA, 1/1/11, 11:15 AM).

  • Ensures authentication by the physician is complete prior to acting on orders entered into the medical record.


  • High school diploma or equivalent GED required. Previous experience of one to two years in a medical office or health care facility preferred.

  • Must have excellent interpersonal and communication skills to interact effectively with customers and all members of the health care team. Is able to perform a variety of duties characterized by frequent change.

  • Must be comfortable operating in a collaborative, shared leadership environment.

  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Mercy and Trinity Health.
