Trinity Health Registered Nurse - Home Care - Part Time - Days in Mason City, Iowa

Provides individualized nursing care/assessments at the home/residence of the client diagnosed with acute, chronic and complex conditions.
What you will Do:
Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.
Provides individualized care to clients with complex care needs including pediatrics, geriatrics, and mental health patients.
Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care. Demonstrates the ability to care for clients with a variety of diagnosis’ including but not limited to orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs.
Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage.
Delivers nursing care for the clients in their homes, utilizing appropriate available resources.
Involves the client/caregiver in goal development and plan of care development.
Involves the client/caregiver in developing and carrying out the plan of care. Educates client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning. Evaluates and documents client's physical and emotional reactions to treatments, care, and medications in the electronic medical record (EMR).
Part Time - 40hrs a pay period, Occasional weekday/weekend call rotation, including holidays.
Current state Registered Nurse License or permit from State Board of Nursing required.
Completion of ADN required BSN preferred.
Must have demonstrated competency in an acute patient care setting for at least one to two years.
Home Health, community health, or hospice experience preferred.
Previous experience with mental health patients preferred, but not required
