What You’ll Be Doing:

Evolent Care Partners’ works directly with primary care organizations across the country to improve patient outcomes and reduce avoidable health care costs. The Practice Performance Coordinator will play a critical role in executing Evolent Health’s mission by contributing to the delivery of the valued-based strategy with Evolent Health’s partners and by contributing to initiatives to growth the department and team. This individual serves a key role of organizing the Performance team to best meet the needs of Evolent Care Partners’ national network. This role will work in collaboration with the Practice Performance Advisors and subject matter experts to support performance across the network.

This role is designed to support all aspects of practice transformation to ultimately ensure the achievement of the Triple Aim and network success in Value-Based Care while also allowing for opportunities to grow into more responsibilities over time.


  • Support the performance team through facilitating network communication and follow through

  • Help partner practices access and effectively utilize Evolent Care Partners’ proprietary tools to succeed in Value-Based Care through platform user provisioning and tracking of change requests related to Data Management and operations

  • Coordinate the interactions between the various subject matter experts and the performance team

  • Partner with Performance team leadership to develop and present content at the executive and national level

  • Facilitate the implementation of assigned initiatives to ensure timelines are met and leadership is kept informed of progress

  • Collaborate with ACO team members and subject matter experts to ensure progress towards ACO, regional, practice, and provider improvement goals

  • Support the coordination of regional and national forums to ensure a positive and productive experience for network partners

  • Manage provider and CRM data (contact lists, performance data, EMR data, etc.) entry and maintenance, including documenting and maintaining process and procedures s (standardize documentation process)

  • Update and maintain project delivery tools (e.g., agenda, meeting minutes, project plans, status updates, risks & issues)

  • Support new-hire onboarding process by ensuring appropriate access and training for on-the-job tools

  • Receive assigned tasks and instructions on work products

  • Ad hoc data entry and organization projects, including document management, file creation/division, report templating, conducting quality assurance and manual entry of data

  • Identify opportunities for improvement of operational efficiency, develop standard work and assess outcomes of change

  • Work independently on routine project tasks and obtain guidance on escalating issues when necessary

  • Develop and maintain a positive professional relationship with project team members and other stakeholders

  • Prioritize competing requirements and validate with supervisor

  • Contribute to key internal initiatives as needed

The Experience You’ll Need (Required):

  • Must have a valid Driver's License and ability to travel regionally or nationally up to 25% of the time

  • We currently require all employees who may be in an Evolent office or partner site to be vaccinated against COVID-19. This is a work from home role that requires travel to partner sites and occasional travel to company office(s) for meetings, therefore, vaccination is required

Finishing Touches (Preferred):

  • Bachelor’s degree

  • 1-3 years of work experience

  • 1-2 years working in healthcare

  • Project management experience

  • Process improvement, lean, six sigma, root cause analysis

  • Experience with Advanced APMs, ACOs, Value-Based Care, and Population Health

  • Proficiency in Microsoft Office products (Outlook, Word, Excel and PowerPoint)

  • SharePoint experience

  • Excellent written and verbal communication skills

  • Demonstrated organizational skills

  • Ability to work and collaborate across multiple teams

  • Desire to learn and develop within Value-Based Care

  • Ability to thrive in group environments

  • Ability to effectively communicate with diverse parties and stakeholders

  • Ability to learn quickly, openness to learn new tools and processes

  • Good sense of how to prioritize multiple tasks at once

  • Desire to work in a fast-paced and entrepreneurial environment that often experiences change

Technical Requirements:

Currently, Evolent employees work remotely temporarily due to COVID-19. As such, we require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router. These at-home technical requirements are subject to change with any scheduled re-opening of our office locations.

Compensation Range: The minimum salary for this position is $50,000, plus benefits. Salaries are determined by the skill set required for the position and commensurate with experience and may vary above and below the stated amounts.
