The ALS Association Care Services Manager in Des Moines, Iowa

  • Information and Referral

  • Serve as primary Chapter contact with ALS clients and their families for the Central/Western portion of Iowa and clients who attend the University of Iowa Clinic in Johnston and/or Des Moines VA ALS Clinic

  • Provide information and educational materials and correspondence

  • Provide virtual home visits as core service to provide education and resource connections; and on rare occasions, if deemed to meet the requirements for an in-person home visit, travel to the client’s home to complete an in-person visit

  • Provide phone and email support

  • Coordinate implementation of Care Services programs available in region

  • Document engagements with ALS clients and families to assist with related care services reports

  • Resource Group

  • Assist Eastern Iowa Care Services Manager in facilitating virtual statewide resource group

  • When deemed appropriate, facilitate in-person gatherings for individuals impacted by ALS to connect and share resources on a periodic basis

  • Equipment Loan Program

  • Assess and identify individual need for durable medical equipment or assistive communication devices in conjunction with other professionals and policies

  • Coordinate delivery and set-up of equipment through equipment loan provider to individual/family

  • Coordinate input of deliveries and equipment returns into inventory system with provider

  • Coordinate equipment donation process, tracking and acknowledgement with provider

  • Work with the Director of Care Services to evaluate and improve quality and inventory of Equipment Loan Program

  • Work with the Director of Care Services to organize, inventory and consolidate Equipment Loan Program inventory at least once yearly at each Equipment Loan Program location

  • Outreach, Provider Relations and Educational Training

  • Actively build referral base and relationships with relevant healthcare providers and other professionals who serve people with ALS in region

  • Promote and provide in-service educational trainings for various provider and community groups

  • Attend University of Iowa ALS Clinic in Johnston monthly and provide in-person education and support to clients, families and healthcare professionals about our services and supports

  • Coordinate with Des Moines VA healthcare professionals via post-clinic phone call to assist in serving and supporting Veterans impacted by ALS

  • Coordinate promotional strategies to serve increasing number of patients in region

  • Other Responsibilities and Activities

  • Provide content and input for Care Services newsletter and other publications as requested

  • Collaborate with Development staff to promote fundraising and other special events as appropriate

  • Actively participate in ongoing training related to ALS and professional skill development

  • Promote and participate in Association’s advocacy/public policy efforts as requested

  • Provide input on Care Services strategic planning as requested

  • Other business and meetings as needed and requested

  • Represent self on behalf of The ALS Association in a professional manner to all constituents and promote the vision, mission, purpose, and values of The Association at all times

    SUPERVISORY RESPONSIBILITIES:

  • This job has no supervisory responsibilities.

    QUALIFICATIONS:

  • Bachelor’s Degree required, preferably in Social Work or related field; commitment to lifelong learning

  • Minimum of 2-4 years of related professional experience, with particular emphasis in medical social work preferred

  • High degree of resourcefulness and knowledge of community resources; great assessment and problem-solving skills

  • Strong coping skills and empathy; ability to strike good balance between personal connection and professional relationship

  • Strong verbal, interpersonal and written communication skills; comfortable making presentations and meeting with a variety of people and organizations to build and sustain impactful relationships

  • High degree of self-initiative, motivation, and discipline; ability to work independently in remote office

  • Strong computer/technology skills; includes MS Office, database utilization, and report/query generation

  • Ability and willingness to travel in state, primarily in central/western Iowa; minimal out of state travel required (valid U.S. driver’s license required). Schedule will be flexible with primary goal to meet position needs

  • Candidate must reside within 30 miles of Des Moines, IA (or willing to relocate)

  • This position will work out of a home-based office

  • Availability and willingness to work evenings and weekends for special projects and events when necessary
