Cardiac Clinical Coordinator:

ESSENTIAL FUNCTIONS :

Coordinates and manages the care of this high risk population through the continuum; including but not limited to evaluations, education, treatment, and follow-up care

Assists in formulating the AHF-VAD goals, performance metrics, policies, procedures, and practice guidelines with the IHC physicians and surgeons

In collaboration with the divisions of cardiothoracic surgery and cardiology, develops an evidence-based comprehensive plan of treatment for patients with AHF-VAD, through clinical pathways, algorithms, protocols and standing order sets

Plans, conducts and evaluates educational activities regarding VAD implantation to VAD patients, families, professionals and the community

Facilitates communication between Shared Care facility and Implanting Center

Serves as a VAD therapy resource to all members of the healthcare team, patients and their families.

Ensures compliance with CMS standards, national guidelines, patient satisfaction, and cardiac registry for best practice

Works with physicians and staff to effectively manage referrals and process patients.

Educate and correspond with outlying facilities and community services regarding care of VAD patients

Shares call coverage

Coordinates VAD research activities

Responsible for coordinating the Joint Commission (JC) disease specific certification process

Assure data collection and analysis for ongoing performance improvement and compliance with the INERMACS registry

Monitors VAD coding and reimbursement

Liaison to mechanical assist device companies to secure and maintain appropriate equipment, supplies, and costs

MARGINAL FUNCTIONS:

Demonstrates awareness and adherence to safety and legal requirements established at the hospital and department levels

Demonstrates self-directed learning needs, and seeks ways to meet own professional development and responds to new clinical and management demands

Maintains confidentiality of patient, personnel and institutional information

All other duties as assigned

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED :

Currently licensed by the State of Iowa as a registered nurse with 5 year acute care or advanced heart failure care

Bachelor’s degree required.

Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.

Basic Life Support (BLS) for the Healthcare Provider certified or obtained by the end of the orientation period (approximately six (6) weeks). Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.
