Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Clinic Support Worker, you will provide interpretation and translation services for patients in the medical clinic, room patients and take vitals, and serve as a resource for staff and patients about community resources.

    What You'll be Doing

  • Performs basic clinical duties such as rooming patients, obtaining vital signs, height, weight, and collection of basic patient intake questions and data in the clinic or on the telephone as assigned. Participates in initial data gathering on patients and goals using tools in the EMR and other tools such as PRAPARE,the PHQ-9 and other tools as instructed..

  • In conjunction with other team members, responsible for follow up on identified issues, such as: referrals for other services, assisting patients in setting and keeping appointments, setting expectations for patients about their follow up appointments, clarifying and ensuring comprehension of care instructions and serving as a liaison to ensure patients are connected and engaged with their care team.

  • Provides interpreting and/or translating services as requested.

  • As directed by the professional clinical staff, reports normal results and pertinent information to patients.

  • Maintains documentation of patient contacts in compliance with PHC policies. Follows organizational procedures/protocol for the organization and management of the electronic medical record (i.e. test results, retrieval, filing, creation, repair, changing of electronic medical record).

  • Acts to facilitate interdisciplinary communication. Utilizes other members of the health care team in assessing and analyzing patient data. Assesses learning needs of the patient and significant others. Gives accurate and complete information to patient and family.

  • Provides assistance to patients and their families to identify needs and facilitate access to community resources and entitlements. . Refers patients to the Health Benefit Specialist when appropriate.

  • Responds to referrals for assistance from and facilitates referrals to PHC clinics, community agencies, and other resources.

  • Prepares exam rooms in accordance with established infection control protocols.

  • Provides cross coverage for other clinic functions within the scope of the position and for which competency has been determined.

  • Maintains supply inventory and stocks supplies in various work areas.

  • Demonstrates current level of knowledge of various payor regulations by functioning within those guidelines.

  • Utilizes appropriate procedures, including the use of PPE (Personal Protective Equipment) in performance of patient care duties.

    Qualifications You Need to Bring

  • High school diploma or equivalent

  • One year of experience in human services, healthcare, interpretation, translation, or related field

  • English and Spanish verbal and written language proficiency, including medical terminology

  • Basic Life Support (BLS) for Health Care Providers Certification *

  • Mandatory Reporter Certification *

    • Must be obtained with Introductory Period if not current.

    Your experience and skills determine your base pay. The hiring range for this position is typically $15.50 - 19.50 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $23.25 per hour.
