Job Summary

The Care Coordinator assists in planning, organizing, developing and directing the day-to-day functions of the nursing services and resident care activities in accordance with the Nursing Home Director to ensure that residents get the best care possible.

Essential Duties and Responsibilities

• Assists the Nursing Home Director in conducting the day-to-day functions of the nursing services in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

• Participates in facility surveys made by authorized government and private agencies as requested by Administrator or the Director of Nursing Home.

• Assists with coordination of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care as needed.

• Coordinate and organize resident care activities and sharing of information among all of the participants concerned with resident’s care to achieve safer and more effective care. Ensures that the resident’s needs and preferences for health services and information sharing.

• Resident- and family-centered, team-based activity designed to assess and meet the needs of residents, while navigating effectively and efficiently. Identifying potential gaps in meeting residents’ needs.

• Works closely with front line staff to assist in care plan compliance.

• Shares hallway charge nurse responsibilities as assigned.

• May work as a floor nurse / supervisor when needed.

• Participates in the development, maintenance, and implementation of Quality Assurance Programs.

• Communicates professionally and respectfully with supervisors and co-workers in a way that generates a collaborative effort and positive team spirit. Optimum resident care requires a positive team effort.

• Attends and participates in continuing education programs designed to keep abreast of changes in the profession, as well as to maintain current licensure.

• Performs other duties as necessary or assigned.

• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

• Complies with all applicable state and federal regulations.

• Attends all mandatory meetings and programs.

• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

• Maintains the confidentiality of patient, resident, employee and organizational information.

• Participate in on-call schedule and responsibilities.

Qualifications

Position Requirements, Licenses, Registrations, Certifications

• RN with state license required; Current license to practice nursing in the state.

• Two years of experience as a professional nurse. Nursing Home experience preferred.

• Basic Life Support certification.

• Ability to create a caring, compassionate relationship with the patients and families.

• Excellent written and verbal communication skills.

• Ability to collaborate with the healthcare team to use evidence-based practice in providing care.

• Must possess effective time management skills.

• Ability to carry out assignments with minimal supervision and guidance.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Has the ability to write routine reports and correspondence. Speak effectively before customers, patients, families and employees of the organization.

While performing the duties of this job, the employee is regularly required to stand; talk and hear; and use hands to finger, handle or feel. The employee must frequently lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, depth perception and ability to adjust focus. May be exposed to infectious diseases and occupational dangers like accidental needle pricks. Must be able to withstand physical and emotional stress.
