  • Position Summary

  • Under direction the Quality Improvement Coordinator assumes accountability and oversight for the Mary Greeley Home Health and Hospice QI programs. Responsible for coordination of quality measurement processes, data analysis and identification of improvement opportunities. Works collaboratively with interdisciplinary staff internal and external to the organization in the resolution of identified or potential problems. Ensures all actions taken in carrying out responsibilities support patient centered care.

  • Reviews medical records to ensure documentation/service is in compliance with DNV, CMS, and Medicaid guidelines and standards.

  • Facilitates and coordinates preparation activities for DNV and other regulatory survey activities.

  • Provides oversight, coordinates and provides direction to the Hospice and Home Health Department QI program.

  • Ensures QI teams are following the FOCUS-PDCA approach for improvement activities.

  • Coordinates data collection and analysis, collaborates with others to identify problems and solutions creating ownership in decision making.

  • Identifies and assures the provision of educational and staff development needs.

  • Responsible for knowledge of DNV. and Medicare standards and updates as related to Home Health and Hospice, communicates and facilitates changes as directed.

  • Communicates with leadership to review outcomes, solutions and regulatory updates. Summarizes quality management and patient safety activities.

  • Conducts special studies as requested by leadership.

  • Provides individual staff education as during orientation and as requested.

  • Communicates and collaborates effectively, both verbally and in writing, in a professional and tactful manner.

  • Remains knowledgeable in and practices Continuous Quality Improvement philosophies and concepts, and statistical problem –solving tools and techniques.

  • Assists in the development and review of various Standards and Policies and Procedures.

  • Performs other work as assigned

  • Qualifications, Knowledge & Experience

  • Required Qualifications (Including any licensure, certification, education):

  • Licensed as a Registered Nurse and authorized by the State of Iowa to practice nursing.

  • Graduation from a school of nursing.

  • Associates or Baccalaureate degree in nursing with a minimum of one year of clinical experience as a registered nurse in home care.

  • Possession of a current valid driver’s license and must maintain eligibility for coverage under Mary Greeley Medical Center’s insurance policy.

  • Mandatory Reporter training within 90 days of hire and must maintain throughout employment.

  • Organizational Requirements:

  • Maintain stroke education per regulatory requirements.

  • Preferred Qualifications:

  • Bachelors degree in nursing.

  • Professional certification in nursing specialty.

  • Expert knowledge in Home Health and Hospice Conditions of participation

  • Required Knowledge, Skills & Experience:

  • Able to demonstrate knowledge of continuous quality improvement concepts.

  • Proficient in communication skills.

  • Ability to pursue and foster a sense of team and partnership across all disciplines.

  • Must be self-directed, flexible, and able to adapt to change.
