Our Home Health team provides services to support clients and their caregivers in their homes. Our services range from care for chronic illnesses such as diabetes, cardiac issues, and kidney problems to shorter-term care following an accident or surgery. We also provide care when clients finish a stay in short-term rehabilitation at one of our communities. We are looking for a Clinical Manager to lead a clinical team to ensure client care is coordinated and appropriately managed.

Responsibilities will include:

  • Mentors team members to develop a solid understanding of home health regulations and concepts including but not limited to OASIS measures, homebound criteria, medical necessity, re-certification criteria, admissions to home health, resumption of services, and client satisfaction results.

  • Maintains visit compliance by completing routine co-visits; completes documentation for all co-visits completed.

  • Oversees client safety, manages client and team member assignments, and ensures compliance with documentation and other standards.

  • Identifies areas for team member development and provides education. Actively participates and sits on the QAPI committee. Performs regular chart audits.

  • Completes ADR reviews and insurance denial appeals. Reviews, updates, and signs interim orders and 485s daily.

  • Maintains and schedules new hire orientation plans. Ensures training is completed and documented.

  • Manages team members including interviewing, selecting, and terminating team members; manages performance; monitors productivity and addresses quality and productivity issues; completes performance reviews on time; and manages timekeeping.

  • Travels in all weather conditions and to all areas of the department service area as needed.

  • Participates in a clinical leadership on-call rotation. Acts as backup for clinical nursing on-call when visits must be covered after hours and on weekends.

Candidates should have:

  • Valid unrestricted Iowa driver’s license.

  • Current vehicle insurance.

  • Ability to speak, read, and write in English.

  • Currently licensed as a Registered Nurse in the state of Iowa is required.

  • Bachelor of Science in Nursing preferred.

  • Two years of full-time supervisory or management work experience in a healthcare setting is preferred.

  • One year of full-time experience as a nurse case manager in home health or home hospice is required.

Our salary range for this role is $71,032 - $85,000 / year. The starting pay rate will be based on years of experience. Previous experience as a supervisor in home health or hospice will be required for the upper end of the range.
